Blazeway Care Services is a care management company located in Lithonia, Georgia. We proudly serve Medicaid members through the Elderly and Disabled Waiver Program (EDWP) and SOURCE. Our team of skilled case managers, nurses, and support staff work with families to ensure that care is accessible, respectful, and tailored to each member's needs.
