We provide Personal Support Services (PSS) under Georgia’s EDWP and SOURCE programs, helping seniors and individuals with disabilities live safely and independently at home. 

•  Must be Medicaid-eligible
   
•  Age 65+ or living with a qualifying disability
   
•  Needs help with daily living activities
   
•  Lives at home (not in a facility)

2.  Get a nurse assessment to determine your level of need
    
3.  Choose Blazeway Care Services as your Personal Support Services provider
    
4.  We’ll match you with a trained PCA and start services quickly
